PROGRAM TEAMS:

Services are available to all Evergreen World ADHC participants. All members are assessed individually by our professional multidisciplinary Team on a daily and periodical basis.

Nursing

Best practices of professional nursing care, nurses monitor, supervise, assess, and implement healthcare under the direction of the participant’s primary care physician to maintain a safe and healthy environment for the well-being of the participants.

Social Worker

Accessible 1:1 supportive counseling, case management, referrals to community services such as in-home support services, supplemental security income, Cal-fresh, Adult protective services, housing, transportation, and mental health resource as needed.

Licensed Clinical Social Worker:

Evidence-based psychological and counseling services such as assessment of emotional functioning, development of functional coping plans, coordination of care, mood monitoring, and holistic approach on wellness.

Physical Therapy

Utilizes therapeutic exercise, patient education, and modalities to restore a person’s full functional potential. We focus on improving strength and balance to increase steadiness in gait. This in turns will prevent falls and injury.

Occupational Therapy

Increases upper body strength, endurance and flexibility. Decreases pain on upper body with physical modalities include hot pack and paraffin bath. Promotes independence with activities of daily life, such as dressing, toileting and bathing through individual training, safety education and consultation.

Speech Therapy

Assessment and referral is available to ensure safety for participants with swallowing difficulties, CVA, and GERD.

Nutrition

Registered Dietitian (RD) provides education, and assessment, and has the knowledge of menu planning to meet the nutritional needs of each participant.  .
